How it works
Anyone—individuals, families, caregivers, or professionals—can complete the form.
If you’re referring someone else, please ensure you have their permission.
Our staff look over the referral to understand the person’s situation and needs.
Program Coordinators review these upon submission.
We call the person listed as the initial contact to begin a supportive, low pressure conversation.
Expect a call within 72 hours.
We will establish a time that works for you for an in-person meeting.
During this meeting, we:
- get to know the individual and their needs
- explain our services
- answer questions
- discuss next steps
Submitting the form does not commit anyone to services.
In general, we support individuals who:
- have a life-limiting illness
- are no longer receiving curative treatment
- live within Saskatoon city limits
- have a safe home environment for volunteers and staff
If you’re unsure—submit the form anyway. We’ll help sort it out.
Once eligible, support may include:
- one-on-one companionship
- respite for caregivers
- help with errands or groceries
- transportation to appointments
- grief and bereavement support

- could benefit from companionship, practical help, caregiver respite, or grief support
If you are unsure whether it’s the right time, please reach out—we’re happy to help. If service is not needed at this time, we will have your information when the time is right.
